Centra is proud to offer an exciting new service to the community. This model of healthcare has been in existence for over 30 years throughout the United States. The program focus is on prevention, wellness and the belief that it is better for seniors with chronic care needs and their families to be served in the community whenever possible. In Virginia, the first PACE Center was opened in 2007. Centra PACE is the 5th program in Virginia and is located in a newly remodeled building at 407 Federal Street in downtown Lynchburg.
What is PACE?PACE is a comprehensive program for adults 55 years of age and older who meet the criteria for nursing facility placement, prefer to stay at home and have an assessment indicating that living at home with the support of the PACE program is a safe alternative.
PACE MissionThe Centra PACE mission is to enhance life for older adults with chronic needs by providing compassionate quality care and services. PACE strives to achieve this mission by empowering participants to live in their homes and in the community for as long as it is medically and socially safe.
About PACEPACE is a national program of comprehensive care which promotes independence and the highest levels of functioning while allowing choice and dignity for members and their families.
Comprehensive services are delivered by an interdisciplinary team of professionals made up a primary care physician, registered nurses, a masters level social worker, health aides, transportation personnel, rehabilitation therapists, dietitians and recreation/activity personnel. The team works with participants and their families to develop a tailored plan of care which is monitored closely and updated as needed.
PACE participants can be picked up from their home each morning and brought to the PACE Center.They participate in social activities, eat a hot lunch with friends, and receive medical assistance as needed from a dedicated, compassionate care team.At the end of the day, the PACE van takes the participants back home to enjoy family, friends and the surroundings they know and love.
About our Medical DirectorVerna R. Sellers, M.D., is Board Certified in Geriatric Medicine, Hospice and Palliative Medicine. Previously, she served as Medical Director of Sentara Life Care and the PACE program in Hampton Roads, Virginia. She is a member of the American College of Physicians, American Geriatric Society, American Medical Directors Association and American Academy of Hospice and Palliative Medicine.
Eligibility
ServicesCentra PACE provides a range of services twenty-four (24) hours a day, seven (7) days a week, three hundred sixty-five (365) days a year. Primary medical care from doctors, nurses and therapists is provided at the Centra PACE Center Monday through Friday 7 a.m. to 5:30 p.m.

Participants may be fully liable for the cost of services obtained without the authorization of the PACE Team (except for emergency services).
How do I enroll?Enrollment is simple. Please call 434.200.6516 to set up an appointment. Our PACE team will arrange to meet you at your home to fully explain the program and arrange a visit to the Centra PACE Center.
FeesCentra PACE is a nonprofit program funded through a unique arrangement with Medicaid and Medicare. If you are eligible for Medicaid and Medicare or Medicaid only, there may be no fee to join the program. If you are eligible for Medicare only, there is a set monthly fee involved. If you do not quality for Medicare or Medicaid, private pay options are available to enroll in the PACE program.
